PALS Participation form : Please note, PALS closely follows the UCF Youth Protection Program Standards for UCF CARD constituents to participate in any program/event.
A new application will be required each year. The release waiver is required by the state of Florida. If you have questions concerning the wavier it can be found on line, Florida Statute 744.301.
All forms must be filled out in full for consideration for your child to participate in the program.
